Insurance
We are an out of network provider; a superbill can be provided upon request. We use Reimbursify to make the process easy.
Appointment packages
Two appointment packages are available for families who need consistent care and follow up. All packages are non-refundable and do not expire–your care journey can take some time!
6-visit package
Includes one (1) 90 minute initial evaluation and five (5) 60-minute follow up visits with any provider of Be Well Baby PDX of any service type.
12-visit package
Includes one (1) 90-minute initial evaluation and eleven (11) 60-minute follow up visits with any provider of Be Well Baby PDX of any service type.
